Purpose:
The purpose of The Ohio State University College of Pharmacy PGY1 Pharmacy Residency Program offered in Ambulatory Care Settings is to provide the resident with experience and confidence to: Be change agents who can develop, implement, and deliver patient care; and Pursue practice, leadership, and academic positions in ambulatory care settings. This PGY1 program fosters development of knowledge, skills and abilities needed to provide outstanding direct patient care and to educate current and future practitioners in ambulatory care settings. This program consists of two primary sites with strong pharmacy presence in patient-centered medical homes.

Practice Site:
OSU General Internal Medicine Clinics
The OSU General Internal Medicine Clinics are a network of 7 NCQA tier 3-patient centered-medical homes and the primary outpatient sites for the nationally-recognized Ohio State University Internal Medicine Residency program. The network is part of the CMS Comprehensive Primary Care Plus (CPC+) program, an Alternative Payment Model focused on value-based care and primary care transformation. The pharmacy team at the site is directly involved in the leadership, development, and delivery of the value-based care initiatives.

Pharmacy residents at this site will interact with more than 100 medical residents, 60 attending physicians, and have access to more than 70,000 patients in an interdisciplinary clinic setting. The pharmacy resident has opportunities to work with physicians to design therapeutic plans for the management of chronic disease states such as hypertension, dyslipidemia, diabetes, anticoagulation, asthma, chronic non-malignant pain and depression/anxiety, along with a variety of acute care issues.
Patient interaction is encouraged through collaborative practice agreements, comprehensive medication reviews, transitional care management, anticoagulation management, diabetes and asthma device teaching, and individualized medication counseling.

Providers within the clinic refer patients to an interdisciplinary diabetes management clinic, where, an interdisciplinary team including physicians and pharmacists, provides individualized care, goal setting and disease education to patients with poorly controlled diabetes. Pharmacists follow up between clinic visits to titrate medications and assess patient progress toward goals. Additionally, pharmacists at the OSU General Internal Medicine clinics provide timely care for patients during transitions of care, complete comprehensive medication reviews, and participates in population health initiatives.
The pharmacy resident plays a vital role in providing answers to drug information questions and educating other health care providers on pertinent medication-related issues. The pharmacy resident also develops and delivers educational presentations to medical residents as part of an ambulatory curriculum.
The primary clinics are located at the Martha Morehouse Medical Plaza and Outpatient Care East.

College of Pharmacy:
Teaching and Learning Certificate
This goal of this certificate is to develop and enhance the teaching skills of post-graduate trainees and prepare trainees for future teaching opportunities in the classroom and practice environment. Residents will be provided with opportunities for teaching that may include didactic teaching in large and/or small groups, facilitation of assessment and interprofessional activities, and clinical teaching of experiential students at all levels of learning. Additional activities may include observance of faculty governance, facilitation of peer training, and opportunities for engagement in scholarly activities related to teaching and learning.
