Graduates will be prepared to provide pharmaceutical ambulatory care services to an underserved population in a community family practice setting including but not limited to diabetes, hypertension, and other chronic disease state management. Residents are provided the opportunity to accelerate their clinical skills beyond entry-level professional competencies in patient-centered care and disease state management through a shared visit model and collaborative practice agreement. Upon completion of the program, the resident will be able to provide medication therapy management (MTM) to patients, provide services designed to promote community wellness, provide disease state management, assist and potentially lead clinical program development as well as a dispensing role and optional teaching certificate. Family Health Services does offer a unique environment to its residents in that it is an FQHC. We utilize integrated team-based patient care, project management, and a diverse patient population.
